Five ACCESS Health Centers Awarded PCMH Level 3 Recognition

Access Community Health Network is proud to announce that the National Committee for Quality Assurance (NCQA) has granted five of our health centers with PCMH Level 3 Recognition.

There are three levels of NCQA PCMH Recognition, and each level reflects the degree to which a practice provides standard health care services. This latest recognition means that these five ACCESS health centers are meeting the highest levels attainable with NCQA.

The five sites that were awarded PCMH Level 3 recognition in ACCESS' journey to becoming an outstanding patient-centered medical home are: 

  • ACCESS Grand Boulevard Health and Specialty Center (previously awarded with Level 1 Recognition)
  • ACCESS at Anixter
  • ACCESS Madison Family Health Center
  • ACCESS Blue Island Family Health Center
  • ACCESS Martin T. Russo Family Health Center
