Program Description:
Access Community Health Network is pleased to offer the Chicago Family Case Management Program that provides case management and support services to low-income pregnant women and their children under the age of one.
Services Provided:
- WIC—Nutrition
- Family Planning
- Health Education
- Prenatal classes
- Child Care
- Immunizations
- Behavioral Health
- Teen pregnancy—Early intervention
- All Kids/FamilyCare (Health Insurance)
- Other resources
To be eligible for Chicago Family Case Management you must be:
- Pregnant and/or have a child under one
- Be on Medicaid
- Be low-income
Cost:
The Chicago Family Case Management program is funded by the Illinois Department of Healthcare and Family Services. Case management services are provided at no cost to patients.
