ACCESS provides comprehensive HIV services for all populations. ACCESS' HIV services model focuses on direct care, prevention, education and case management support.

Case Management
All patients with HIV are assigned to a case manager trained in the needs of patients infected with HIV. The case manager works closely with the medical provider to ensure services are coordinated, comprehensive and appropriate to each patient's needs. Our case managers also refer patients for other social services throughout ACCESS' large network of partner agencies.
Prevention Focus
While we provide high quality treatment for those living with HIV, we also work very hard to prevent new infections. ACCESS' prevention specialists provide prevention education to adults and youth in various locations throughout Chicago.
Special Programs
AIDS Foundation of Chicago Case Management Services
Client-centered assessment, case planning, service coordination, referral, advocacy, and follow-up through which we seek to comprehensively meet the needs of persons affected by HIV.
Corrections Re-Entry and Supportive Services
We offer case management and empowerment program for individuals living with HIV/AIDS that have recently been released from incarceration.
Sheriff's Female Program
We provide women in the home monitoring program with sessions on health education, HIV, STD and Hep C prevention, self-empowerment, spiritual care, behavioral health, and referrals to needed health services (every 3rd Tuesday of the month).
S.I.S.T.A.: Sisters Informing Sisters about Topics on AIDS
Afro-centric group program for women to explore issues around HIV prevention, self-empowerment and healthy relationship building in a supportive, confidential, culturally-centered environment.
Youth Empowerment - Prevention and Empowerment program for teen girls
Sessions focus on HIV/AIDS education, self-empowerment, coping strategies, relationship building and other teen issues.
Cost:
All patients are welcome! We accept Medicaid, Medicare, All Kids and most major health insurances. Title I, Title III and Title IV funding may also be available for eligible patients. If you are uninsured, we offer a sliding-fee scale based on ability to pay.
