Program Description:
Access Community Health Network is proud to provide the physicals and immunizations required by the U.S. Citizenship and Immigration Services for immigrants applying for U.S. citizenship. Immigration physicals are provided by appointment only.
Services Provided:
- Review of medical history
- Physical exam
- Lab work
- Necessary vaccines
- TB skin test
- Chest x-ray (if necessary)
It takes approximately four weeks to process the paperwork for Immigration physicals.
Cost:
The cost is $150 for the basic exam and up to $175 for more additional tests. Payment is expected at the time of service.
What you need to bring to the visit:
- Photo ID
- Copy of shot records
